Position Summary:
Heart To Serve Home Care is seeking high-caliber CNAs who demonstrate strong dementia care skills, professionalism, and emotional intelligence. Our caregivers provide relationship-centered, concierge-level care designed to enhance safety, independence, and quality of life for clients living with Alzheimer’s and memory-related conditions.

Primary Responsibilities:
• Provide personal care including bathing, grooming, dressing, toileting, and hygiene support
• Deliver safe ambulation and transfer assistance
• Implement structured dementia communication and redirection strategies
• Monitor behavioral, cognitive, and functional changes
• Support fall prevention and environmental safety
• Assist with meal preparation, hydration, and companionship
• Maintain accurate and professional documentation
• Follow individualized care plans and collaborate with clinical leadership

Required Qualifications:
• Active Indiana CNA license in good standing
• Demonstrated experience supporting clients with dementia or Alzheimer’s disease
• CPR certification and TB screening
• Strong communication and observation skills
• Professional, dependable, and compassionate approach to care
• Reliable transportation
• Ability to pass comprehensive background screening
